In the previous installment, we started an examination of how will NASA know that it has successfully changed its culture to one more focused on safety and the management of risk, as demanded by the Columbia Accident Investigation Board (CAIB)? In this article, we continue our look at some of the issues NASA will be facing in trying to change its culture, the lessons of which can be applied to any organization trying to create a more proactive risk management culture. NASA will know its culture is changing if outsiders can see the following behaviors:
