Catastrophe Disentanglement: What Can You Do When Projects Fail?

Consider this: "We know why projects fail, we know how to prevent their failure -- so why do they still fail?" [5]. These oft-quoted words from Canadian government official Martin Cobb seem to highlight our frustrations with failing projects. In effect, Cobb is asking: "What is it that we are we missing?" Most of us who are involved in software projects would probably agree that we are definitely missing something.
