Software Project Success and Failure: Part I

There is a general belief in the information systems community, among developers, managers, and users, that the failure rate of software projects is unacceptably high. It is not unusual to still hear and see references to a "software crisis" at conferences, despite some important paradigm shifts in thinking that have occurred over the last decade (e.g., the introduction of agile methods and the increasing use of iterative development approaches, which have been expected to reduce project cancellation rates). Has this actually happened?
