Reports on successful projects are archived and ignored. Reports on failing projects are read and studied -- many of us have read quite a number of them. Such reports typically state that projects fail for reasons like "changing requirements," "insufficient planning," "unrealistic estimation," and "poor quality." These "official" explanations are valid, of course.
